The Department of Biliary-Pancreatic Surgery at Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine was established in October 2013. Its predecessor was the Biliary Tract Specialty Group of the Department of General Surgery at Renji Hospital. Renowned Chinese surgical experts, including Professor Lan Xichun, Professor Kuang Yaolin, Professor Shi Weijin, Professor Wang Pingzhi, Professor Zhang Baigen, Professor Chen Zhiping, Professor Wu Zhiyong, and Professor Cao Hui, made outstanding contributions in this field, laying a solid foundation for the establishment of the Department of Biliary-Pancreatic Surgery. Currently, the Department of Biliary-Pancreatic Surgery is a National Key Clinical Specialty, the Shanghai Biliary Tract Disease Consultation Center, a member unit of the Biliary Tract Disease Diagnosis and Treatment Center of Shanghai Jiao Tong University School of Medicine, a member unit of the Pancreatic Disease Diagnosis and Treatment Center of Shanghai Jiao Tong University School of Medicine, a key discipline and characteristic discipline of Renji Hospital, and the location of the Biliary Tract Disease Institute of Shanghai Jiao Tong University School of Medicine (Renji) and the Shanghai Key Laboratory of Tumor System Regulation and Transformation (under preparation). The Department of Biliary-Pancreatic Surgery at Renji Hospital has a comprehensive surgical team proficient in modern surgical techniques including open surgery, laparoscopic minimally invasive surgery, robotic surgery, and endoscopic surgery, benchmarking against the top domestic standards, with its disciplinary influence gradually increasing. Since its establishment, the annual outpatient volume, number of discharged patients, number of surgeries, and number of cases of key diseases and key surgeries have increased year by year. It has maturely carried out various ultra-major and complex surgeries for biliary-pancreatic diseases, with particularly outstanding achievements in the standardized treatment of pancreatic tumors and biliary tract tumors, with over 600 cases of biliary-pancreatic tumor cure per year. The Department of Biliary-Pancreatic Surgery at Renji Hospital was the first in China to advocate the concept of "total mesenteric excision for pancreatic head cancer," providing more patients with the opportunity for radical surgery and improving the R0 resection rate and five-year survival rate; proposed a series of treatment strategies for liver metastasis of pancreatic cancer, improving the overall treatment outcomes for these patients; earlier proposed the treatment concepts of anatomical liver resection and hilar lymph node dissection for intrahepatic cholangiocarcinoma, achieving good treatment results; and was the first domestically and internationally to perform and refine various surgical methods for radical resection of gallbladder cancer, filling several international gaps in treatment concepts. In addition, the Department of Biliary-Pancreatic Surgery at Renji Hospital has conducted individualized treatment research targeting ErbB signaling pathway mutations in gallbladder cancer patients, and through the established gallbladder cancer biobank, has effectively prolonged the survival of gallbladder cancer patients without increasing adverse reactions, serving as a model of translating basic research into clinical practice. The Department of Biliary-Pancreatic Surgery possesses first-class domestic endoscopic and laparoscopic techniques, utilizing laparoscopy, choledochoscopy, and duodenoscopy for minimally invasive treatment of extrahepatic biliary stones; percutaneous transhepatic cholangiography (PTC); endoscopic retrograde cholangiopancreatography (ERCP); percutaneous transhepatic gallbladder drainage (PTGBD); trans-sinus tract biliary interventional therapy; and single-incision laparoscopic cholecystectomy. Additionally, the Department of Biliary-Pancreatic Surgery performs laparoscopic/robotic pancreaticoduodenectomy, distal pancreatectomy, splenectomy, partial hepatectomy, and various abdominal wall hernia repairs (such as inguinal hernia, incisional hernia, parastomal hernia, etc.), with minimally invasive surgeries accounting for over 60% of the total surgical volume of the Department of Biliary-Pancreatic Surgery. The Department of Biliary-Pancreatic Surgery at Renji Hospital combines the concept of enhanced recovery after surgery with day surgery, adhering to the philosophy of "rapid, precise, and minimally invasive," completing 4,000 day surgeries annually, of which laparoscopic cholecystectomy, hernia repair, and ERCP account for over 80% of the total day surgeries, greatly shortening hospitalization time and reducing hospitalization costs. The Department of Biliary-Pancreatic Surgery has now formed a series of research directions in biliary tract tumors and pancreatic tumors. It was the first internationally to successfully construct animal and cell models of gallbladder cancer and the first to construct a mutation spectrum of gallbladder cancer, as well as the first internationally to pioneer targeted therapy for gallbladder cancer. At the same time, it has established a clinical specimen bank and database, and carried out various clinical studies including the application of three-dimensional visualization in the perioperative period of biliary tract tumors, the clinical application of total mesenteric excision for pancreatic cancer, conversion therapy for advanced pancreatic cancer, strategies for hilar lymph node dissection in intrahepatic cholangiocarcinoma, research on the extent of radical resection for gallbladder cancer, and research on the metabolic causes of gallstones. Since its establishment, it has undertaken over 60 research projects, including 31 national-level and 13 provincial/ministerial-level projects; and published over 100 SCI papers. Currently, the Department of Biliary-Pancreatic Surgery has 47 beds in the East Campus, focusing on biliary-pancreatic and abdominal wall hernia diseases, including various biliary tract diseases (gallbladder and intrahepatic/extrahepatic bile duct stones, choledochal cysts, gallbladder cancer, bile duct and ampullary cancer, etc.), pancreatic diseases (pancreatic cysts, pancreatic endocrine tumors, pancreatic cancer, etc.), and various inguinal hernias, incisional hernias, etc.; 24 beds in the West Campus, focusing on retroperitoneal tumors and abdominal wall surgical diseases, including liposarcoma, leiomyosarcoma, malignant mesothelioma, and various inguinal hernias, incisional hernias, hiatal hernias, etc.; and 51 beds in the South Campus, admitting various general surgery diseases, including benign and malignant hepatobiliary-pancreatic diseases, gastric, small intestine, and colorectal tumors, benign and malignant thyroid and breast tumors, various abdominal wall hernias, and poor wound healing, etc. The Department of Biliary-Pancreatic Surgery currently has 45 full-time clinical physicians, including 26 with senior professional titles, 2 doctoral supervisors, and 9 master's supervisors.
