Department Details

Since its establishment, the Prenatal Diagnosis Center of the International Peace Maternity and Child Health Hospital has enjoyed a high clinical and research status in Shanghai and throughout the country. Our center is equipped with outpatient clinics, an operating room, and laboratories, and undertakes various prenatal diagnosis examinations and treatments in Shanghai. It is a department that closely integrates clinical practice, scientific research, and teaching into a medical-education-research entity. Our hospital's Prenatal Diagnosis Center specializes in the diagnosis and treatment of various high-risk pregnancies and genetic diseases. With interventional prenatal diagnosis techniques as the premise, cytogenetic and serological Down syndrome screening as the foundation, and ultrasound and MRI as auxiliary tools, we have further developed prenatal molecular genetic diagnosis, non-invasive maternal blood DNA screening, and high-throughput sequencing technologies to serve the majority of pregnant women and their families. 1. Outpatient Clinics Our center has prenatal diagnosis consultation clinics, genetic clinics, medication clinics, and consultations. Consultations include prenatal diagnosis consultations and perinatal consultations. The outpatient clinics are located on the third floor of Building 2 of the hospital. The prenatal diagnosis consultation clinic integrates high-risk pregnancy consultation, prenatal and postnatal care consultation, and prenatal consultation. In October 2012, it reopened with a brand-new appearance to receive couples from all over the country, receiving more than 7,000 consultants annually. Genetic counseling mainly targets pregnant women with a family history of genetic diseases. Based on the clinical symptoms of the disease and the genetic characteristics of the family pedigree, combined with molecular diagnostic methods, it confirms and prevents genetic diseases. The medication clinic mainly targets pregnant women who use medications during pregnancy. Based on the type, timing, dosage, and combined effects of medications, it assesses the safety of drugs for the pregnancy and fetus and provides reasonable consultation for pregnant women. Our center collaborates with clinical obstetricians, geneticists, and pharmacists from the pharmacy department to provide detailed and professional consultation for pregnant women using medications during pregnancy. The two major consultation centers collaborate with chief physicians from various tertiary hospitals in Shanghai, from multiple departments including obstetrics, neonatology, cardiac surgery, urology, neurosurgery, imaging, genetics, and family planning, to provide multidisciplinary and comprehensive diagnosis and consultation for high-risk pregnant women. 2. Surgery Our center has 3 clinical surgeons, including 1 deputy chief physician, 2 senior attending physicians, and 2 nurses. We perform first-trimester chorionic villus sampling, second-trimester amniocentesis, and third-trimester umbilical vein puncture. First-trimester chorionic villus sampling can be performed at 11-14 weeks of gestation, mainly for pregnant women with increased NT or fetal hydrops indicated by first-trimester ultrasound, as well as those at high risk based on early serological screening, achieving early diagnosis and early intervention in prenatal diagnosis. Second-trimester amniocentesis has been performed in our center for more than 30 years, with the appropriate gestational age being 18-23 weeks. The procedure is performed skillfully, mainly for diagnosing pregnant women at high risk based on second-trimester serological screening, those with a history of adverse pregnancy outcomes, advanced maternal age, family history of genetic diseases, and those exposed to teratogenic substances during pregnancy. In recent years, the miscarriage rate of amniocentesis in our hospital has dropped to 0.1%, far lower than that reported by international peers. Third-trimester umbilical vein puncture mainly targets pregnant women who have missed the window for amniocentesis, those with abnormal or malformed findings on ultrasound screening, and other high-risk pregnant women. Our center has been performing umbilical vein puncture for nearly ten years with skilled technique, and in recent years the statistically recorded miscarriage rate is only 0.5%, far lower than international reports. The center can also perform intrauterine treatments such as amnioinfusion, amnioreduction, fetal reduction, intrauterine transfusion, and radiofrequency ablation. 3. Laboratories Our center's laboratories include a prenatal screening laboratory, a prenatal diagnosis laboratory, a molecular diagnosis laboratory, and a high-throughput sequencing laboratory, with 1 associate researcher, 1 deputy chief physician, 4 supervising technicians, and 4 technicians. The prenatal screening laboratory mainly performs first-trimester and second-trimester serological screening, combined with first-trimester NT, to screen for Down syndrome, trisomy 18, and open neural tube defects. Our center conducts more than 10,000 first-trimester and second-trimester serological screenings annually. The prenatal diagnosis laboratory revolves around interventional prenatal diagnosis techniques. Based on the genetics laboratory, it performs first-trimester chorionic villus culture, second-trimester amniotic fluid culture, third-trimester umbilical cord blood culture, and fetal chromosome karyotype analysis. Currently, more than 1,000 cases of prenatal chromosome karyotype analysis are performed annually. With the strong support of the cytogenetics laboratory, our center has also developed molecular diagnosis and screening technologies mainly based on fluorescence quantitative PCR, gene chips, and high-throughput sequencing. The center is now equipped with the Agilent gene chip platform (Agilent SureScan Microarray Scanner), mainly targeting pregnant women with a history of adverse pregnancy outcomes or with fetal malformations detected by ultrasound screening but with normal fetal chromosome karyotype analysis. The high-throughput sequencer (Illumina Next SeqTM 500) currently equipped in the center is one of the most advanced high-throughput sequencers in the world, mainly providing assistance for high-risk pregnant women, those with a history of adverse pregnancy outcomes, those with a family history of genetic diseases, and those with difficult cases.
