The Cerebrovascular Disease Center of Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine was officially established in May 2021. The establishment of the Cerebrovascular Disease Center is not only an important measure for the hospital to deepen healthcare system reform and implement the "14th Five-Year Plan" for medical science and technology development, but also a response to the people's demand for health, and is of milestone significance for improving discipline construction. The Cerebrovascular Disease Center was jointly established by expert physicians specializing in cerebrovascular diseases from neurology and neurosurgery. It adheres to a patient-centered and disease-oriented integrated scientific diagnosis and treatment model for stroke, breaks down traditional disciplinary boundaries, optimizes the allocation of disciplinary resources, and is a clinical diagnosis, treatment, and research platform for cerebrovascular diseases integrating "medicine, education, research, prevention, and management." At present, the Cerebrovascular Disease Center of our hospital has two wards and one neurological intensive care unit (NICU) ward in the South Campus, with 48 open beds, an independent 24-hour Cerebrovascular Disease Center emergency department, a half-day center general outpatient clinic five days per week, and specialized outpatient clinics for stroke prevention and screening, cerebrovascular intervention, and headache and post-stroke cognitive impairment. There are currently 17 physicians, including 3 chief physicians, 3 associate chief physicians, 7 attending physicians, and 4 resident physicians, all with postgraduate degrees or above. The center is fully equipped with video electroencephalography, ambulatory electroencephalography, electromyography, ambulatory electrocardiography, ambulatory blood pressure monitors, color Doppler ultrasound diagnostic instruments, transcranial magnetic stimulation devices, pneumatic therapy devices, centrifuges, ultra-low temperature freezers, and other equipment. Under the leadership of Director Wan Jieqing, the discipline leader, the center is guided by the needs for prevention and treatment of cerebrovascular diseases and adopts the internationally advanced comprehensive stroke center diagnosis and treatment model. It has established a green rescue channel for acute cerebrovascular diseases and has reached a relatively high international level in stent treatment of intracranial and extracranial arterial stenosis, interventional treatment of complex intracranial aneurysms, minimally invasive surgical treatment of cerebral hemorrhage, and comprehensive treatment of cerebrovascular diseases. Its "integrated, standardized, and process-based" stroke diagnosis and treatment model is bringing benefits to patients. Since its establishment nearly 3 years ago, the center has continuously improved team building and encouraged every physician to actively innovate and forge ahead. On the basis of deep integration of neurology and neurosurgery physicians, it has established three cerebrovascular disease intervention and surgical teams, a neurocritical care team, and a comprehensive treatment group team. In coordination with the cerebrovascular disease team of the Department of Neurosurgery in the East Campus, it has advanced in parallel to carry out various highly difficult surgeries, gradually forming a cerebrovascular disease treatment system with Renji characteristics. As of the end of December 2023, the East and South Campuses admitted more than 4,000 patients annually, with nearly 60,000 outpatient and emergency visits, more than 250 emergency thrombectomy treatments for acute stroke, more than 200 emergency interventional treatments for aneurysmal hemorrhage, and more than 2,000 surgical procedures including minimally invasive intervention or clipping for intracranial and extracranial arterial stenosis or occlusion, cerebral hemorrhage, and cerebral aneurysms, and cerebrovascular bypass, significantly improving the survival rate and quality of life of patients with cerebrovascular diseases. In terms of scientific research, under the leadership of Director Wan Jieqing, the center has actively participated in multiple clinical and basic research projects, applied for and obtained multiple National Natural Science Foundation and national-level projects, published multiple papers in internationally first-class journals, and won the Third Prize of Shanghai Medical Science and Technology Award in 2022 (first completer). In terms of new technologies, the center has carried out TCD and other vascular ultrasound monitoring before and during interventional procedures to safeguard against perioperative complications, and has actively applied for the promotion and application of new technologies. In terms of academic exchange, since its establishment, the Cerebrovascular Disease Center has held the "Pujiang Cerebrovascular Disease Forum" and other academic exchange activities every year, providing an academic exchange platform for scientists and researchers in the field of cerebrovascular diseases, discussing the current research frontiers in basic and clinical medicine related to cerebrovascular diseases and related disorders, accelerating innovation and translation in the prevention and treatment of cerebrovascular diseases, and promoting the vigorous development of the discipline. In terms of certification and training, the center has obtained certification from the National Pharmacological Base and Medical Device Base, and all physicians in the center have obtained GCP certificates. In 2021, the center also obtained certification as one of the first batch of National Neurointervention Training and Training Bases, has trained dozens of visiting physicians from multiple domestic hospitals, promoted cerebrovascular disease treatment methods, and its influence is continuously radiating across the country. In terms of clinical cooperation, the center carries out cross-hospital clinical cooperation and, together with the Eye & ENT Hospital of Fudan University, performs internal carotid artery embolization through various surgical methods for patients with nasopharyngeal carcinoma invading the internal carotid artery, thereby reducing the possibility of intracranial massive hemorrhage and significantly increasing patients' 3-year overall survival rate and tumor progression-free survival rate from the previous 27.7% and 18.9% to 69.1% and 33.9%, respectively. In terms of grassroots education, the center has also established medical alliance cooperation with multiple rehabilitation hospitals, regularly conducts community education, opens specialized outpatient clinics, and performs teaching rounds. The center's various science popularization efforts have also made good progress, regularly updating the "How Much Do You Know About Stroke" public account, and has won multiple awards in science popularization lectures jointly with community hospitals.
