Department Details

The Department of Oncology at Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine is among the fourth batch of national key clinical specialty construction departments, an FDA-approved clinical trial base for new anticancer drugs, one of the first batch of Shanghai standardized wards for cancer pain, a Shanghai Jiao Tong University civilized team, and a doctoral program in oncology. It features chemotherapy, targeted therapy, and clinical research of new drugs for gastrointestinal, breast, pancreatic, and other solid tumors with high incidence in China. The Department of Oncology currently has 81 beds (Oncology Wards I and II, and Breast Medical Oncology), with over 10,000 patient admissions annually. Five major优势 characteristic diseases (gastric cancer, colorectal cancer, breast cancer, esophageal cancer, and biliary-pancreatic tumors) account for more than 85% of cases. The physician team consists of 14 members, with 85.7% holding master's or doctoral degrees and over 50% having overseas research experience. Over the past 3 years, the department has led or participated in 1 National Science and Technology Support Program project titled "Development of Molecular Marker-Based Early Diagnosis, Molecular Subtyping, and Targeted Therapeutic Drugs (Drug Candidates) for Gastric Cancer," led 3 National Natural Science Foundation projects, and had 4 person-times selected for various talent programs including Shanghai Outstanding Academic Leader and Shanghai Medical Leading Talent. More than 20 SCI papers have been published. The department has participated in 1 international clinical trial, led 2 domestic multicenter clinical trials as the lead unit, and participated in 4 domestic multicenter clinical trials including phase I clinical studies. Clinical Features Under the leadership of discipline leader Professor Zhu Zhenggang, department head Zhang Jun leads the team with the goal of building a "research-oriented clinical department," distilling and forming the "three orientations" concept of precision oncology (goal-oriented therapy, biomarker-oriented therapy, and tumor biological behavior-oriented therapy). The department promotes whole-course management and full-symptom control for cancer patients, and has established and refined a system for patient assessment, decision-making, and efficacy evaluation. The Department of Oncology maintains a leading position domestically in clinical diagnosis and treatment, with subspecialties such as gastrointestinal oncology and breast oncology reaching or approaching international advanced levels, and some achieving international leading levels. In the past 3 years, the number of hospitalized patients with common malignant tumors such as gastric cancer, colorectal cancer, and breast cancer has gradually increased. The department emphasizes improving medical quality management, having formulated and refined medical standards and a quality management system for oncology, and improved and implemented standardized diagnosis and treatment of common tumors. Rational drug use is promoted, with preference for national essential prescription drugs, earning the department the honor of Shanghai Advanced Unit for Rational Drug Use. While providing adequate basic medical services for the public, the department has enhanced its disciplinary influence and reach. Despite rapid growth in ward workload, the average length of stay has been maintained at nearly 3 days and bed utilization rate at nearly 125%. While business volume continues to grow, the department maintains strict standards for civilized service and quality nursing, earning patient recognition and the 2014 Shanghai Jiao Tong University School of Medicine Civilized Team award. Through further creating distinctive features and with the goal of being rooted in the Yangtze River Delta, serving China, and going global, the department has authored or co-authored 4 diagnostic and treatment guidelines for gastric cancer, colorectal cancer, etc., organized by the National Health and Family Planning Commission and the Chinese Society of Clinical Oncology (CSCO), and has established a remote visual consultation and telemedicine technology platform. The Department of Oncology has also established long-term partnership agreements with Dali University Affiliated Hospital in Yunnan, Zunyi First People's Hospital, Dujiangyan People's Hospital, and Fujian Union Hospital, conducting academic support through short-term personnel dispatch, regular MDT case demonstrations, and remote consultation academic weeks. While emphasizing multidisciplinary, standardized, individualized, and innovative anti-tumor treatment, the department also carries out standardized palliative and symptom-reducing treatment for advanced tumors and is among the first batch of "Shanghai Good Pain Management (GPM) Demonstration Wards." To optimize palliative oncology care and improve patient quality of life, the department has appointed 2 full-time physicians and 4 full-time nurses for cancer pain management, conducts regular monthly cancer pain education, has added麻醉药箱 and immediate-release morphine tablets for cancer pain titration in the wards, operates a cancer pain follow-up hotline, and has independently developed and launched the "随身小护士" app, effectively achieving "routine, quantitative, comprehensive, and dynamic" cancer pain management. It is currently also a CSCO antiemesis demonstration ward. The Department of Oncology closely relies on the hospital's traditional优势 diseases including gastrointestinal tumors, breast tumors, and pancreatic tumors, regularly offering or participating in 10 tumor MDT clinics for gastric cancer, colorectal cancer, breast cancer, pancreatic cancer, etc., with long-term regular operation, whole-course management, and rationally structured standardized protocols. Clinical New Technologies (I) Development of Gastric Cancer Screening Technology Suitable for China's Conditions and Establishment of High-Incidence Field Sites To align with the national strategy of advancing the frontier of cancer prevention and treatment, the Department of Oncology collaborated with Shanghai Xinhua Hospital Chongming Branch (formerly Chongming County People's Hospital) and the Shanghai Center for Disease Control and Prevention to establish gastric cancer screening field sites in high-incidence areas of Shanghai including Chongming, Changxing, and Hengsha, and explored ultrasound gastric cancer screening technology using oral ultrasound contrast agents as controls, accumulating experience for establishing appropriate screening technologies for gastric cancer—a high-incidence cancer in China—in rural and underdeveloped areas. To date, population screening has been completed in nearly 20 natural villages with over 10,000 individuals, and interim results were published in Digestive and Liver Disease in 2017. On this basis, a directed referral system for gastric cancer patients in the Chongming area was developed and established, refining the whole-course management process from early screening, preoperative staging, comprehensive treatment, to community follow-up. Simultaneously, a gastric cancer tissue, body fluid, and metabolite specimen bank and clinical database were established and refined, exploring molecular subtyping of gastric cancer to lay the foundation for early diagnosis, prognostic assessment, and individualized medicine for gastric cancer. (II) Exploring Novel Tumor Drug Treatment Models Represented by Metronomic Chemotherapy The Department of Oncology primarily treats patients with advanced or progressive tumors; therefore, shifting from the previous "eliminating all tumor" approach to "living with the tumor" is of great significance for prolonging survival and improving quality of life in patients with advanced gastrointestinal tumors. Leveraging the basic research strengths of the Shanghai Key Laboratory of Gastric Tumors, the department first clarified in basic research the anti-tumor mechanism of metronomic capecitabine chemotherapy through inhibition of tumor angiogenesis, and then accumulated clinicopathological data, establishing the department's leading position nationally in conducting multicenter clinical research on metronomic chemotherapy for gastrointestinal tumors. The Department of Oncology led the first multicenter clinical study of metronomic chemotherapy in China (BIG-MOON study; NCT03158610), with some advanced cancer patients' chemotherapy paradigms shifting from "maximum tolerated dose" to "minimum effective dose," and participated as one of four Chinese representatives in drafting the Asia-Pacific Expert Consensus on Metastatic Colorectal Cancer (2014). (III) Individualized and Precision Therapy for Solid Tumors Through national key clinical specialty construction, the department has preliminarily distilled and formed the academic philosophy of "goal-oriented therapy," "molecular marker (panel)-guided individualized therapy," and "biological behavior-guided individualized therapy." Since 2014, in collaboration with the Department of Laboratory Medicine, the department has comprehensively implemented full RAS gene mutation testing and UGT gene polymorphism testing in clinical practice for patients with advanced colorectal cancer to guide targeted drug treatment regimens and adjust chemotherapy drug doses; both technologies have been included in Shanghai's medical insurance testing items. The department collaborates with the Shanghai Key Laboratory of Gastric Tumors to archive blood and peritoneal drainage fluid specimens from patients treated in the department, providing ample clinical sample support for basic research on related diseases. The department authored two editions of the "Chinese Expert Consensus on the Treatment of HER-2 Positive Gastric Cancer," participated in drafting three national standards issued by the Standardization Administration of China (GB/T30989-2014, 20121377-T469, 20131210-T-469), and participated in the compilation of the "Asia-Pacific Expert Consensus on the Treatment of Metastatic Colorectal Cancer." (IV) Clinical Research of New Drugs and "Clinician-Initiated Research Based on Clinical Questions" In support of the construction of the FDA anticancer new drug clinical research base, the department has participated in multiple international and domestic clinical studies, including "An Open-Label, Randomized, Controlled, Multicenter Phase II Clinical Study Comparing Cetuximab Combined with FOLFOX4 versus FOLFOX4 Alone as First-Line Treatment in Chinese Patients with RAS Wild-Type Metastatic Colorectal Cancer," "A Prospective, Randomized, Double-Blind, Multicenter Registration Clinical Study of Nimotuzumab Combined with Gemcitabine versus Placebo Combined with Gemcitabine in the Treatment of K-RAS Wild-Type Locally Advanced or Metastatic Pancreatic Cancer," etc. The department also undertakes Phase I and II clinical studies of multiple domestically developed novel drugs. The department, in collaboration with优势 clinical specialties and partner hospitals (Chinese PLA General Hospital, Xiangya Hospital of Central South University, West China Hospital of Sichuan University, Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology), leads the multicenter
